SERVICE AGREEMENT FOR WALKING/DROP-IN VISITS Walking/Drop-In Visits Service Agreement This Service Agreement sets forth the terms and conditions of your agreement with Schreiber Tails, LLC (“Schreiber Tails”) regarding walking and/or drop-in services for the following pet(s): * Please read and check each paragraph, then sign and date below. I attest that my animal(s) is healthy and fully immunized, as required by local, state and federal law. I understand that care for my pet will be provided according to the instructions given on the Client Intake Form. Any special requests have been explained in detail. I understand and agree that my pet will be allowed to interact with Schreiber Tails staff, and I attest that I have provided an accurate statement regarding any past aggressive behaviors towards people or other pets. In the event that my animal causes injury to another animal or to a person while in the presence of Schreiber Tails staff, I agree to assume all liability. I am responsible for my own animal’s actions and agree to pay for any damages my animal may cause. I hereby waive and release Schreiber Tails, its employees, trainers and agents from any and all liability of any nature for any injury, death, illness or the loss of my animal while in the custody of Schreiber Tails staff AS LONG AS REASONABLE ACTION IS TAKEN BY SCHREIBER TAILS STAFF TO AVOID HARM. I understand and agree that Schreiber Tails staff may impose restrictions or requirements on terms of walking or drop-in visits, or rescind access to future services due to behavior issues. I agree to discuss any concerns with Schreiber Tails within 24 hours after service. I agree to pay the amount due, in full, by the service start date. I agree to allow pictures of my animal(s) and me for posting on Schreiber Tails website, publication in promotional materials and/or for other marketing or informational purposes without remuneration.
